Nurse Practitioner — Integrative Primary Care (0.5-0.75 FTE)

The opportunity
We are a small, independent, provider-owned integrative primary care practice in Bend. We provide comprehensive primary care and layer functional and integrative therapeutics into that care, working to address root causes rather than manage symptoms.
We are hiring a nurse practitioner for 2-3 clinic days per week (0.5-0.75 FTE). Unfortunately we are not able to hire PAs for this role, as the position practices independently under Oregon’s full practice authority for nurse practitioners.
This is a good fit for a clinician who wants longer visits, a manageable panel, and real autonomy — and who is curious about integrative medicine whether or not they have formal training in it yet.
Compensation

A guaranteed salary plus a share of what you collect. We think this is fairer than straight salary and more predictable than pure production. You are never below the floor, and above it you keep a share of every dollar.

Base salary: guaranteed, paid every two weeks regardless of pt volume

Productivity share: % of collections on services you render

You are paid: the greater of the two — the share replaces the base once it exceeds it

What that works out to
Our compensation model combines a guaranteed base salary with a percentage of collections, giving you a predictable floor with additional earning potential as your patient panel grows.

This allows you to earn as much as $94/hr equivalent even after factoring in 5 weeks PTO.
For context, Bend’s market average for a full-time nurse practitioner is roughly $72/hr, this compensation structure is designed to be competitive with Bend-area and regional health-system NP compensation, while offering the flexibility and autonomy of a small independent practice.

The table assumes you code at close to the level our established providers do. Our billing is not exotic — mostly office visits, new patient codes, and wellness visits. You inherit our payer contracts and workflows on day one, so the only variable in your hands is accurate documentation and code selection.

Collections lag encounters by 30 to 90 days. Expect base salary only for your first two to three months, with the productivity share arriving after that. We reconcile every two pay periods and hand you the underlying report every time. No black box.
The base is a floor, not an advance to be repaid. If collections come in low in a given period, you keep the base. Nothing carries forward, and you never owe us money.

What the work looks like

Full-spectrum primary care for commercially insured, Medicare, and Medicaid populations, including:
-
All things primary care 6 yo and older.
-
DM, HTN, Anx/Dep, acute concerns.
-
Prescription management
-
Women’s health, men’s health, pediatrics, and mental health
-
Bioidentical hormone replacement for menopausal care and men’s health
-
Autoimmune screening and management, thyroid disorders, POTS, gut health
-
Medically supervised weight management, including GLP-1 therapy
-
Ordering, interpreting, and managing advanced imaging and testing
-
We do not do chronic opioid management or Controlled substance treatment for ADHD but will comanage with pain medicine and Psychiatry.

Mentoring is available in any of these areas. We would rather teach a curious clinician than hire someone who already knows everything.

You will also be invited — not required — to contribute to occasional patient education, blog writing, and community outreach.
What we are looking for;
Required
-
Active, unrestricted Oregon NP license with prescriptive authority, or eligibility to obtain one, plus DEA registration
-
National certification in family nurse practitioner
-
Strong documentation habits and a willingness to code accurately — this matters, and it is part of how you are paid
-
Comfort working autonomously while genuinely enjoying a small team
-
Willingness to learn functional and integrative approaches, and to keep learning
Preferred
-
2+ years in primary care, internal medicine, or urgent care
-
Experience with bioidentical hormones and menopause care
-
Working knowledge of supplements, nutrients, and nutrition as they support specific conditions
-
Prior exposure to functional or integrative medicine
